Trigger
Stage 3 begins when Stage 2 / 2.a signals show:
- continued worsening drift
- confirmed risk patterns
- or escalation criteria defined by the organization
Exact escalation thresholds are TBD (configurable) per organization and clinical policy. The system does not assume universal risk cutoffs.

At Stage 3, providers commit to:

Stage 3a — Review Required
- Provider reviews the signal set
- Determines next step based on clinical judgment
Stage 3b — Soft Contact
Soft contact is non-voice follow-through, including:
- text or app outreach
- portal message
- scheduling an appointment without voice-to-voice review
Scheduling a follow-up without provider discussion is soft contact.
Stage 3c — Hard Contact
Hard contact is voice-to-voice, telehealth, or in-person engagement.
Used when:
- clinical judgment indicates higher acuity
- specialty data confirms escalation risk
